I understand that yoga is a physical activity that includes stretching, balance, and mindful movement. I acknowledge that participation is voluntary and I believe that the activities involved are safe for myself and my child(ren). I understand that it is important to inform the instructor of any health concerns or physical limitations prior to or during class, and I will notify Zeny Grace Ingal immediately if there are any changes in my family’s health or physical condition that may affect participation. *

This includes any incidents occurring before, during, or after the classes.
